Prescription Form for Patient Kit Requests

Download and complete the physician recommendation form with the required prescription information. The patient is required to fill in the required information located at the top of the form. When the patient contacts us, we will send the kit directly. Alternatively, you can fax the form to 206-219-4000 with the patient's insurance information, mailing address, and phone number, and we will handle the rest.

Request a Buccal (Cheek) Swab Kit
