How will Tamoxitest be paid for?

There are two modes of payment:

• Self Pay: You may order and pay for the test yourself, but a physician prescription is still required in the U.S. The discount patient pay price is $295 based on advance payment. Genelex will provide you with documents needed to seek payment from your insurance company. If you have us bill your insurance company in some cases the patient pay price may be less than required insurance co-pays. You may also wish to pay for the test in advance if you are uncomfortable with your DNA test results becoming part of your permanent medical record.
  
  
• Insurance Pay: If you want your insurance company to pay for the test it must be ordered by your doctor. Once your order is received, a cheek swab DNA collection kit with directions will be sent to you. Please use the return mailer to submit samples and results will be sent in ten days or less. Accelerated testing is available for an additional charge. Call a DNA testing consultant at (800) 523-3080 for details and to order.
